Job Description

The purpose of positions in this classification is to plan, organize and facilitate medically prescribed music therapy activities for patients. Develops treatment plans with personalized goals and objectives for motor skills, social/interpersonal development, comfort, self-awareness, etc.. Travels to patient homes and performs music therapy care in accordance with accepted standards of practice.

  • Travels to patient homes and performs music therapy care in accordance with accepted standards of practice. Assists in developing a treatment plan based on the evaluation with objective goals to be approved by the physician. Implements treatment plan. Observes, records, and reports the patient’s reaction to music therapy and changes in condition. Revises the plan of treatment as appropriate. Evaluates outcomes of care.
  • Instructs patients, family members of patients and other health personnel in phases of therapy with which they may work with patient.
  • Coordinates on-going care with the Hospice RN Case Managers and other clinical staff to ensure coordination of care. Reports any concerns or issues. Identifies any barriers to achievement of identified goals/outcomes and works in collaboration with the Hospice RN Case Manager and other patient care team members to assure coordination of care is provided to attain desired outcomes.
  • Contacts and consults with the physician and Home Care Clinical Manager and/or RN Case Manager regarding the plan of care. Communicates significant changes in patient condition. Provides regular updates on progress and recommends revision of plan care as relates to occupational therapy.
  • Documents visit notes, treatments, and plan. Ensures visits are completed in a timely manner. Follows-up to ensure notification and documentation of missed visits is provided in accordance with proper procedures in a timely manner.
  • Assists in the development and utilization of music therapy materials to educate staff, patients and families, and others regarding music therapy.
  • Attends and participates in mandatory meetings, in-services, and conferences.
  • Attends safety training sessions and ensures for compliance with safety rules and procedures. Assists in ensuring for patient safety. Identifies and notifies manager of potential safety hazards. Reports any work-related accidents and injuries in a timely manner. Complies with the procedures for documenting and submitting leave and light duty time.
  • Maintains and increases knowledge of the music therapy field.
Minimum Education & Experience:

Bachelor’s degree in Music with a music therapy emphasis from a program approved by the American Music Therapy Association required with two to three years progressively responsible music therapy experience. Current board certification (MT-BC) by certification board for music therapists (CBMT) required. Hospice experience preferred.


Knowledge, Skills & Abilities:

This position requires travel to conduct music therapy. Ability to utilize musical instrument(s) and other devices for playing/making music. Ability to reference music principles, concepts and methodology. Ability to research various music resources. Knowledge of generally accepted music therapy standards.Knowledge of (or ability to develop knowledge of) Medicare and Medicaid regulations, requirements and reimbursement criteria. Skilled in (or ability to become skilled in) electronic patient charting system (HER).Must maintain a valid WI driver’s license, acceptable driver’s record, & proof of current auto insurance required.


Physical Demands:
This position typically rates as LIGHT WORK as defined by the Dictionary of Occupational Titles but can be heavier if a larger instrument is utilized.
